ATTENDANCE |

Excellent attendance in Faith Formation program is essential to the continuous faith formation and the continuity of the learning experience for students. Therefore, all students are expected to attend every class.
Excellent Attendance is defined as missing 2 or less classes per semester in Faith Formation sessions and no absences in a sacrament preparation process

Note: An excused absence is still considered an absence from class.
- Although the absence is still counted, missed classes can be ‘made up’ by completing all missed work.
- When classes are missed - the chapter work and any assignment from the catechists must be completed to make up the lost class lesson.
- Children in a sacrament year, must complete the lessons to make up the absence to be eligible for sacrament preparation.
- In the event of severe illness with prolonged absences, in addition to all work being made up, the student must meet with the catechist and the director.
- It is the parents’ responsibility to contact the Faith Formation Office to inform them of any extenuating family circumstances that may impact a child’s attendance.
- Habitual absences will affect eligibility for entry into sacrament preparation processes.
- Due to the concentration of the curriculum, students may not miss a class or activity in a Sacrament Preparation process.
- Please be punctual in bringing and picking up your children. If you have an emergency that will cause you to be late in picking up your elementary child/children, please call the Parish Office at 602-938-4227 ext 20 (after 5:00 pm on Tuesdays) to let us know you will be late
SIGN IN/SIGN OUT PROCESS |

- For the safety of the students in grades 1 - 6, parents and/or the designated adult must escort students to their classrooms and sign them in and out of every class. There is no ‘curb service’. A meeting will be set up with the parents of any child who is repeatedly not signed in or out of class to determine corrective measures. If someone other than the child’s parent will be picking him/her up from class, we must receive a note from the parent providing us with that person’s name and relation to the child.
- A note or email (through their parents) will be sent to children when absent to let them know that they were missed in class and given a home assignment to complete at home before the next class. If you receive an email in error please call or email faithformation@olvstr.net so we may amend our records.
- For a well-rounded faith formation experience, children will not be allowed to have the same catechist.
- Students and/or parents are not allowed inside any class or meeting room without a catechist present.
- Students must never be left unattended by parents or catechists.
- Students should arrive no earlier than 10 minutes prior to the start of class nor linger after class without the catechist or designated adult to secure the safety of the children at these times.
- For security reason, classroom doors will be locked when the class starts.
LATE PICK UP OF STUDENTS |

Catechists are not able to secure the safety of the children as they complete their classroom duties. Therefore, students not picked up within 5 minutes after class is over will be taken to the on-site (Religious Education Office) place to wait for their parents.
Note: If parents and/or emergency contact person cannot be reached after 30 minutes, Child Protective Services and/or the local law enforcement agency will be called.
- After waiting 10 more minutes, parents will be called on their cell phone. If no response is received, the office will call the emergency contact number(s).

EMERGENCY ALTERNATE PICK UP PROCEDURE |

Students will be released only to the designated persons on the registration form. When an alternate person must sign out students, parents must inform the Faith Formation Office, preferably through an email and when not possible via a phone call.
- When dropping off the student for class, the parent must also provide a note clearly stating who will pick up the child/children. An official picture ID with the person’s name printed on it, such as a driver’s license, will be required from the person picking up the child/children.
